In most commercial interiors, millwork has to survive use. In healthcare it has to survive cleaning — repeated, aggressive, chemical cleaning, several times a day, for fifteen years. That single difference drives nearly every material and construction decision on a healthcare casework package, and it is the one most often missed at specification.

What actually fails in healthcare casework

Healthcare casework rarely fails structurally. It fails at the junctions:

  • Edges and seams — the first place disinfectant penetrates. An edge band that performs fine in retail will lift under a hospital cleaning regime.
  • Countertop-to-backsplash junctions — an open joint here is both a cleaning failure and an infection-control finding.
  • Sink surrounds — constant moisture plus chemical exposure; the highest-failure location in any clinical package.
  • Toe kicks and floor junctions — where mop chemicals and floor scrubbers do their damage.
  • Hardware — hinges and pulls that corrode, or that are impossible to clean around.

This is why solid surface dominates clinical work: it can be seamed invisibly, thermoformed into coved backsplashes and integral sinks with no crevice at all, and it is renewable — sanded and repaired in place rather than replaced.

Accessibility is a millwork problem, not a layout problem

Accessible requirements land directly on the casework, and they change the construction beneath the counter — not just its height:

  • Accessible counter sections at a lowered height, with adequate length
  • Knee and toe clearance, which dictates the entire cabinet build-up below — this is what gets missed when the elevation is designed first
  • Reach ranges for anything a patient or resident operates
  • Approach and clear floor space, coordinated with the layout rather than assumed

Requirements vary by jurisdiction. Get the accessible portion into the shop drawings early — it is one of the most common causes of late redesign on a healthcare package. (See code compliance.)

Material emissions in occupied care settings

Healthcare and education specifications routinely limit formaldehyde and VOC emissions — because the people in these buildings are, by definition, least able to tolerate poor indoor air quality. That means no-added-formaldehyde or ultra-low-emitting composite cores, adhesives and finishing systems within specified limits, and documentation retained and supplied rather than merely claimed. What to put in a healthcare casework specification

  • The cleaning protocol and chemicals the casework must withstand — state them; “commercial grade” tells a fabricator nothing
  • Solid surface vs. laminate by location, with seam and coving requirements
  • Integral vs. drop-in sinks, and radii at coved junctions
  • Accessible sections: which units, what height, what clearance
  • The emission standard for cores, adhesives and finishes — named, not implied
  • Hardware grade and corrosion resistance
  • AWI quality grade per element
